15 team members attended this incident alongside colleagues from Greater Manchester Police.

Team members assisted GMP with their enquiries and ultimately used a stretcher to carry the person 400m across the moor to a waiting NWAS ambulance.

As the evening progressed, GMP instigated a search of the immediate area, which involved local Divisional Officers, NPAS Warton and GMP Dog teams.

In to the early hours of Thursday 13th August, the search was gathering momentum when sadly the body of the missing woman was located alongside a small lake in the Martland Hill area.

GMP at 02.54hrs paged the team for our immediate assistance in recovering the woman’s body from this location, which was at the base of a short but steep wooded slope at the edge of the lake.

Our Team Leader Garry Rhodes MBE liaised initially with Police and Officers on scene, resulting in a full call out page at 03.26hrs.

At this stage a small group of team members had already declared their immediate availability, and so a quick response was possible.

Our first team members arrived at the Martland Hill Lane RVP at 04.14hrs, and soon enough members were on hand to follow the Police Officer who was awaiting our arrival, to the incident location.

By 05.10hrs with due consideration given the sad circumstances of this incident, we commenced the evacuation of the woman’s body back to the RVP.

This incident concluded on scene at 05.43hrs with the last of our team members leaving the RVP leaving for home as a full and sunny dawn beckoned.

The pager call was answered by our Deputy Team Leader Chris Greenhalgh, who obtained further details of the incident from GMP.

At 10.55hrs the team was paged to be available later, whilst in the interim Chris and colleague team member Mike Marsh visited the quarry to liaise with GMP on scene, and assess what was required regarding the evacuation of the males body.

Available team members gathered at our Ladybridge Hall Base / HQ, and were then paged to meet up with Chris and Mike at circa 12.30hrs, on Cox Green Road, giving access to Cox Green Quarry.

It is our understanding that the body of the male had been located earlier this morning by a GMP Officer, the team being contacted shortly thereafter.

The man’s body was evacuated from the quarry by team members using one of our specialist mountain rescue stretchers, with this sad incident concluding on scene at circa 13.52hrs.

Team members present also assisted GMP with the provision of a cordon in the area.

• Greater Manchester Police: Force Control Room, Divisional Officers, CID and PCSO’s.
• Bolton Mountain Rescue Team: All four of our Land Rover Mountain Rescue Ambulances attended, with thirteen team Call Out list members present on scene, and two further members stood down responding.

A full team call out was paged at 14.58hrs after further details had been sought from Lancashire Constabulary by team Call Out Contact Mike Marsh.

The incident was to assist Police with the recovery of a male’s body, sadly discovered by members of the public earlier this same afternoon, in Mean Brook, Ooze Castle Wood, Pickup Bank, Belthorn, Darwen.

The team responded to undertake a joint operation with Lancashire Constabulary Divisional assets and the North West Police Underwater Search and Marine Unit.

Initially our first responding members liaised with Police on scene, determining safe access and a safe evacuation route for the man’s body.

Working on scene with Officers from the North West Police Underwater Search and Marine Unit, the 30 minute mountain rescue stretcher evacuation from the location commenced at 17.40hrs, following routine investigations conducted on scene.

By circa 18.50hrs our members had departed on scene back to our garage and home locations.

• Lancashire Constabulary ; Divisional Officers, CID Officers, SOCO
• North West Police Underwater Search and Marine Unit
• Bolton Mountain Rescue Team ; Four Team Land Rover Mountain Rescue Ambulances, 18x team call out list members.

It has subsequently been confirmed by Lancashire Constabulary, that subject to formal identification, it is believed that the body is that of a 51 years old Accrington man, who has been missing since Wednesday 9th April 2014.

Since the man’s disappearance, a major Police search operation and multiple media appeals have been mounted.

Alongside the continuing Police search operations and enquiries, searches for the missing man, have also been conducted on at least six separate occasions by our colleagues at Rossendale and Pendle MRT, assisting Lancashire Constabulary, on April 13th through to April 30th in many areas.

The condolences of those in Bolton MRT, Rossendale and Pendle MRT and Lancashire Constabulary are extended to the family of the man involved.

With team members already gathering at our Ladybridge Hall Base/HQ to depart for the City Airport event, two team members were despatched in Team Land Rover BM3 to give their assistance.

Chris Greenhalgh and John Dickinson met with two Detective Officers and a CSI Officer, at the junction of Greens Arms Road and the A666 at 10:05.

Here the three Officers were transported in our BM3 Land Rover along the Cadshaw track (A locked access track) to the scene of the incident which occured in the early hours of this morning.

The campsite of the persons involved in incident 81/2013 was located, alongside the continuing evidence gathering of how the man involved in the incident came to fall over the edge of Yarnsdale Delf Quarry, sustaining fatal injuries.

Officers were escorted by our two members to the top of the quarry, and to other relevant locations in the area.

By 11:59 their investigations on scene were completed, and the three Lancashire Constabulary Officers were transported back along the track to their waiting vehicle on the A666.

All departed the scene at 12:15.

The pager call was answered by our Team Leader Garry Rhodes MBE, who was informed that the accident was off the A666. Based upon our Team Leader’s extensive local knowledge, and the information passed to him, he was able to inform Ambulance Control that the likely incident location was the quarries or natural rock outcrop in the Cadshaw area, and that local access tracks were gated and locked but that the team had access keys.

At this early stage of the incident two team members were already responding to our Ladybridge Hall Base / HQ and Bolton Central Fire Station garage locations to collect team vehicles.

At 00:44 a fully detailed pager call out was made to all team members, with some members already responding at this time to the general area of Turton and Entwistle Reservoir.

At the same time, the first Police had arrived at the A666 RVP. They were soon followed by the responding NWAS Ambulance, but neither could gain vehicle access to the track to the Cadshaw area, due to the gate being locked. They they set off on foot (A fifteen minute brisk walk – if you know where to go)

At 00:44 our first team Land Rover went mobile from our LBH Base / HQ, followed by our second team Land Rover which went mobile from GMFRS Bolton Central Fire Station Garage at 00.50.

At 00:50 our first team member John Fletcher arrived at the A666 RVP, joined at 00:54 by team member Alan James, then at 00:56 by John Dickinson.

All three team members set off down the same access track.

Also at 00:56 our first responding Land Rover (BM3) arrived at the roadside RVP of the A666 near its junction with Greens Arms Road, near to the Turton and Entwistle reservoir.

Traffic on this main route between Bolton and Darwen is always busy, and given that the road is unlit and traffic generally is very fast (this is the exact location of an RTC hotspot) great care had to be taken with safety aspects of the Ambulance, Police and MRT vehicles parked there.

At 00:59 our third responding Land Rover (BM1) went mobile from our LBH Base/HQ.

At 01:00 our Team Leader Garry Rhodes MBE arrived at the RVP along with our second responding Land Rover BM2.

With the access gate now unlocked by the team, our BM2 vehicle, Team Leader’s vehicle and BM3 vehicle all accessed the track, collecting John Fletcher, Alan James and John Dickinson along the way.

At circa 01:00 our BM2 vehicle with five members on board arrived at the casualty site, the first arriving MRT personnel at the actual incident location.

A group of family and friends had been camping above the site of the Yarnsdale Delf Quarry, also known as Cadshaw Quarry or Main Quarry.

It was reported ot us that at some point in the night/early hours of the morning, members of the group had gone to a nearby “rave”, but had become separated on returning to their campsite.

It was after becoming separated and searching the immediate area that that one member of the group located the mising injured male, a man in his twenties from the Darwen area. He was found at the base of Yarnsdale Delf Quarry, amongst a large rock boulder field, with multiple major injuries, consistent with a fall from height, and the emergency services were then alerted.

Prior to the team’s arrival at the casualty site, a Police Officer who was first on scene had commenced CPR, being joined by the NWAS Emergency Ambulance crew.

Our five first responding members lent their immediate assistance to the Police and NWAS crews on scene, with our Team Leader arriving at the incident scene at 01:10 followed by our BM3 vehicle at 01:11.

Emergency casualty care continued, including CPR, and the team also deployed specialist PELI Portable Emergency Scene Lighting units.

By 01:15 our BM1 vehicle was at the roadhead and also deployed along the track.

The severely injured man was then placed on a long board, and his serious leg and ankle fractures were splinted using the team’s specialist vacuum splints.

He was then placed in our BM3 Land Rover whilst CPR continued, and was transported back to the road head.

At this stage responding Fire Fighters from LFRS Darwen also arrived at the casualty site, to be stood down on scene.

The A666 was now totally closed to all traffic with traffic queues quickly forming even at this early hour of the morning, but with a urgent casualty transfer necessary and a large volume of responding emergency vehicles and personnel in one area, it was fully required to do so for safety reasons. (Many thanks to the overall vast majority of drivers and passengers who were waiting and understood the reason for this, but no thanks at all to the very small number who complained at Police and ourselves when the road was reopened)

At the road head the casualty was transferred to the Emergency Ambulance, with CPR being maintained throughout.

The injured man’s partner and a member of the camping group had also been transported to the road head by our Team Leader in his vehcile.

The NWAS Ambulance departed at circa 01:46 for Royal Blackburn Hospital under Police escort, the Police also transporting the man’s partner to hospital.

Team member Michael O’Brien travelled on board the NWAS ambulance, assisting the two rear crew/NWAS personnel with continuing casualty care.

Back at the incident scene, team members assisted Lancashire Constabulary Officers with their initial investigations.

We were then informed that after admission to Royal Blackburn Hospital, which had been put on standby to receive the injured man, the male involved had died of his significant injuries.

By 03:30 the last team members departed the incident road head location (See also Incident 80, and Incident 81)

It appears that the man involved in this tragic accident had somehow fallen from the top of Yarnsdale Delf Quarry onto the large rock boulder field below, a vertical fall of some 75 feet/22.8 metres.

The team’s condolences are expressed to the partner of the man involved, his young child, family and friends.

At he conclusion of this incident a passing taxi driver alerted remaining team members to a collapsed man nearby on the main Bolton to Darwen road. Members immediately responded along with our BM2 vehicle to discover an intoxicated male asleep in a dangerous location on the pavement, immediately next to this busy road.

He was awoken and informed us he was happy to walk home. (This is recorded here for information only and does not feature in our Incident figures)

• NWAS (Lancashire) Broughton EOC, Control Room staff.
• NWAS (Lancashire) Emergency Ambulance from Accrington Ambulance Station, three crew members on board, including a Student Paramedic.
• Lancashire Constabulary – Four Darwen and Blackburn based Officers with their vehicles, and one CID Officer.
• Lancashire Fire and Rescue Service – One Pump Appliance from Darwen Fire Station and five on board crew members.
• Bolton Mountain Rescue Team – Eighteen Team Call Out list members on scene, four specialist Mountain Rescue Land Rover Ambulance vehicles.

For all the emergency service responders involved this was a very serious and intense rescue incident, with all services present working seamlessly together on what was ultimately a very sad outcome.

With the team being in a position to immediately respond and be on scene possibly before the available responding NWAS resources, our BM3 Land Rover Ambulance was despatched to the nearby residential address, arriving on scene shortly thereafter to the report of a male collapsed.

An NWAS responding Response Car then arrived very quickly, soon followed by an NWAS Emergency Ambulance, but sadly the male involved was declared deceased on scene.

Subsequently Rossendale and Pendle MRT were requested to attend this same incident, with some of their members, including their Team Leader Peter Goble, responding directly from the Joint Foundation Course.

At circa 12.20hrs, our Team Leader Garry Rhodes MBE whilst at the same Joint Foundation Course, received a call from Peter Goble requesting that Bolton MRT Water Team members already present at the course, respond to assist RPMRT recover the body from the shallow river that runs through Healey Dell.

Bolton MRT Team members Elaine Gilliland, John Fletcher, John Dickinson and Steve Fletcher, all equipped with full appropriate water PPE (Personal Protective Equipment) departed the Joint Foundation Course for the incident at 12.30hrs in our Land Rover Mountain Rescue Ambulance ‘BM3,’ with RPMRT member Paul Heywood on board acting as navigator to the incident.

Our Deputy Team Leader Geoff Seddon, who heads up our integral water team, also responded collecting our 3.5m semi rigid inflatable SAR Boat from our Ladybridge Hall Base / HQ.

Upon arrival at the incident 13.21hrs, our members with Paul Heywood, met with responding RPMRT members and GMFRS resources, including their Water Incident Unit from Heywood Fire Station.

Very shortly afterwards, at 13.25hrs, our four members present, along with Paul Heywood were stood down as not required, and returned to the Joint Foundation Course.

Our Deputy Team Leader Geoff Seddon was similarly stood down responding.

It is our understanding that the body was recovered by GMFRS resources on scene.
